Dr. Mariam adjusted her silk blazer and checked her reflection in the glass doors of the Maple City Medical Center. The autumn leaves swirled in perfect spirals outside, their reds and golds matching the building's modern copper accents.

"Good morning, Dr. Mariam!" called Rebecca from the reception desk. "Your nine o'clock is here early."

Mariam smiled, her brown eyes sparkling with confidence. She had built her reputation helping clients transform their lives through healthy living. Today felt especially promising.

In the consultation room, she reviewed her notes for Jake Martinez, a twenty-six-year-old who desperately wanted to lose weight before his wedding. This would be their third session together.

"I can do this," Mariam whispered to herself, smoothing her glamorous black waves.

Her phone buzzed with a text from her boss, Dr. Henderson: "Board meeting at four. Need those quarterly results looking perfect."

Mariam's stomach suddenly felt tight. She hadn't eaten breakfast in her rush to look professional. Maybe she'd grab something quick after Jake's appointment.

The Hunger That Hides

Jake Martinez sat across from Dr. Mariam, his wedding photo clutched in his hands. "I've gained three pounds since our last session," he said, his voice shaking. "Maria will notice. The wedding is in two months."

Mariam's stomach growled loudly. She pressed her hand against her blazer, hoping Jake hadn't heard. "Weight fluctuates daily," she said, forcing a smile. "What matters is the overall trend."

"But I've been following your meal plan exactly!" Jake's eyes filled with tears. "I even threw out all the ice cream in my freezer."

Ice cream. Mariam's mouth watered at the thought. She hadn't eaten anything since yesterday's dinner meeting with Dr. Henderson. Her hands trembled slightly as she wrote notes.

"Let's review your food journal," she said, trying to focus.

Jake opened his notebook, revealing detailed entries for every meal. Meanwhile, Mariam's phone buzzed with a text from her neighbor: "Amazing new burger place opened on Maple Street! You have to try their triple-stack special!"

Mariam's stomach growled again, louder this time. Jake looked up, concerned.

"Are you feeling okay, Dr. Mariam?"
